Psychology says people who are scared of visiting a doctor may actually fear what happens before the appointment

For many people, anxiety about visiting a doctor begins long before they enter the clinic. The fear may come less from the appointment itself and more from imagining possible diagnoses, tests or bad news. Psychologists describe this as anticipatory anxiety, the worry or distress experienced while expecting a future event.
